16px;">（一）救治对象：</span></strong><span style="font-size: 16px;">“十三五”期间，对我县农村建档立卡的贫困人口、经民政部门核实核准的孤儿、特困人员和城乡最低生活保障对象(以下统称城乡贫困人口)中，罹患食道癌、胃癌、直肠癌、结肠癌、肺癌、耐多药肺结核、慢性粒细胞白血病、急性心肌梗塞、脑梗死、血友病、I 型糖尿病、甲亢、儿童苯丙酮尿症、尿道下裂及地中海贫血等15种重大疾病的患者进行专项救治。</span></p><p style="text-indent: 2em;line-height: 1.5em;"><strong><span style="font-size: 16px;">（二）定点救治：</span></strong><span style="font-size: 16px;">按照属地、就近要求，原则上选择在县人民医院定点救治。对于县人民医院不具备救治条件的病种，可选择到市级定点医院：</span></p><p style="text-indent: 2em;line-height: 1.5em;"><span style="font-size: 16px;">1、赣州市人民医院。救治病种：食道癌、胃癌、直肠癌、结肠癌、肺癌、慢性粒细胞白血病、急性心肌梗塞、脑梗死、血友病、I型糖尿病、甲亢、儿童苯丙酮尿症、尿道下裂及地中海贫血。</span></p><p style="text-indent: 2em;line-height: 1.5em;"><span style="font-size: 16px;">2、赣南医学院第一附属医院。救治病种：食道癌、胃癌、直肠癌、结肠癌、肺癌、慢性粒细胞白血病、急性心肌梗塞、脑梗死、血友病、I型糖尿病、甲亢、儿童苯丙酮尿症、尿道下裂及地中海贫血。</span></p><p style="text-indent: 2em;line-height: 1.5em;"><span style="font-size: 16px;">3、赣州市妇幼保健院。救治病种：儿童苯丙酮尿症、尿道下裂、地中海贫血。</span></p><p style="text-indent: 2em;line-height: 1.5em;"><span style="font-size: 16px;">4、赣州市第五人民医院。救治病种：耐多药肺结核、食道癌、胃癌、直肠癌、结肠癌、肺癌、脑梗死。</span></p><p style="text-indent: 2em;line-height: 1.5em;"><span style="font-size: 16px;">5、赣州市肿瘤医院。救治病种：食道癌、胃癌、直肠癌、结肠癌、肺癌、慢性粒细胞白血病、甲亢。</span></p><p style="text-indent: 2em;line-height: 1.5em;"><span style="font-size: 16px;">6、赣州市中医院。救治病种：脑梗死、I型糖尿病、甲亢。</span></p><p style="text-indent: 2em;line-height: 1.5em;"><span style="font-size: 16px;">7、赣南医学院第三附属医院。救治病种：食道癌、直肠癌、结肠癌、肺癌、急性心肌梗塞、脑梗死、I型糖尿病、甲亢、尿道下裂、地中海贫血。</span></p><p style="text-indent: 2em;line-height: 1.5em;"><strong><span style="font-size: 16px;">（三）救治程序：</span></strong><span style="font-size: 16px;">救治对象凭“二级以上医院出具的疾病诊断证明书、户口证明、参保证明、贫困证明”等，到县卫生计生委（县城石田心长宁街7号）填写《城乡贫困人口重大疾病专项救治审批表》，申请办理大病专项救治。患者办理入院免费救治时，需提供《城乡贫困人口重大疾病专项救治审批表》、病历资料、身份证、户口本、参保证明、民政救助证明等相关资料。</span></p><p style="text-indent: 2em;line-height: 1.5em;"><strong><span style="font-size: 16px;">（四）费用结算。</span></strong><span style="font-size: 16px;">在县人民医院和其他具备即时结算条件的市级定点医院救治的城乡贫困患者，医疗机构实行“先诊疗、后结算”服务和“一站式”结算服务，患者结清自付部分费用后即可办理出院。在不具备即时结算条件的定点医院救治，由患者垫付救治费用后，回县“一站式”服务窗口按规定报销。</span></p><p style="text-indent: 2em;line-height: 1.5em;"><span 16px;">①对持有我县常住户口，患有急性淋巴细胞白血病、急性早幼粒细胞白血病、急性原粒细胞部分分化型白血病等3个病种的0-14周岁（含14周岁）白血病儿童实行免费救治。</span></p><p style="text-indent: 2em;line-height: 1.5em;"><span style="font-size: 16px;">②对持有我县常住户口，患有先天性房间隔缺损、先天性室间隔缺损、先天性动脉导管未闭、先天性肺动脉瓣狭窄、法洛四联症、先天性主动脉缩窄、先天性肺静脉异常引流等7个病种的0-14周岁（含14周岁）先心病儿童实行免费手术治疗。</span></p><p style="text-indent: 2em;line-height: 1.5em;"><strong><span ②先心病：江西省人民医院、南昌大学第一附属医院、南昌大学第二附属医院、江西省儿童医院、赣南医学院第一附属医院、南昌市第一医院、解放军第九四医院、九江市第一人民医院、九江学院附属医院、解放军第一七一医院、赣州市人民医院、赣州市立医院、宜春市人民医院、上海远大心胸医院。</span></p><p style="text-indent: 2em;line-height: 16px;">对持有我县常住户口、参加城镇居民基本医疗保险和新型农村合作医疗的贫困家庭（城乡低保、农村五保、城镇“三无”人员、福利院供养孤儿、新增四类优抚对象和低保边缘户）患有精神分裂症、分裂性情感障碍、偏执性精神病、双相（情感）障碍、癫痫所致精神障碍等5类重性精神病患者实行急性期住院和缓解期门诊免费救治，其中缓解期门诊免费救治每例定额标准为每人每月200元。</span></p><p style="text-indent: 2em;line-height: 1.5em;"><strong><span style="font-size: 16px;">2、定点医院：</span></strong><span style="font-size: 16px;">①急性期住院：赣州市第三人民医院、寻乌定安医院。②缓解期门诊免费救治：赣州市第三人民医院、寻乌县人民医院、寻乌定安医院。</span></p><p style="text-indent: 2em;line-height: 1.5em;"><strong><span style="font-size: 16px;">3、救治流程：</span></strong><span style="font-size: 16px;">持“疾病诊断证明书、身份证（或户口本）、医保卡及监护人身份证（或户口本）”到县卫生计生委（县城石田心长宁街7号）填写《重大疾病免费救治审批表》，签订“免费救治协议书”，→持“审批表”到县民政局、医保局等部门审批，→持“患者身份证（或户口本）、医保卡、审批表、免费救治协议书及监护人身份证（或户口本）”到定点医院接受住院或门诊治疗。</span></p><p
